The Louisiana Pharmacists Association is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education.

LPA will be tentatively offering a total of 11.0 live contact hours for this convention.  All continuing education materials will be made available through the association website prior to the convention.  Printed materials will only be provided as requested on the registration form.

Target Audiences

Continuing education activities are noted on the agenda with a Universal Activity Number (UAN) ending with a P, T, or P/T.  Those ending with P have a target audience of pharmacists only.  Those ending with T have a target audience of pharmacy technicians only.  Those ending with P/T target both pharmacists and pharmacy technicians.

Credit

LPA will approve all activities individually for the noted amount of credit hours. All activities will expire by July 31, 2019.

Successful Completion

To receive credit for continuing education activities, participants must complete the CE Report Form, marking each continuing education activity attended.  This information must match all sign-in sheets for each session.  These sign in sheets will only be available at the beginning of each session.  Participants shall also complete an evaluation form for each activity.

Faculty Disclosures

Faculty and all others who have the ability to control the content of continuing education activities sponsored by LPA are expected to disclose whether they do or do not have any real or apparent conflict(s) of interest or other relationships related to the content of their presentation(s).  It is not assumed that these financial interests or affiliations will have an adverse impact on faculty presentations.  There are no conflict(s) of interest for any of the activities.

Financial Support

No financial support has been obtained for any of the educational activities.

Statement of Credit

All activity participation will be reported directly to CPEMonitor by August 5, 2019.  No statements of credit will be distributed.

Post-Convention Evaluation

LPA strives to deliver quality educational programming to better enable pharmacists and pharmacy technicians to further develop the profession.  In an effort to evaluate the effectiveness of these activities, LPA may contact participants periodically to evaluate the implementation of the tools and resources provided through the educational activities.
