Award Nominations

Each year, LPA honors members of the Pharmacy community who exemplify the high professional standards embodied by the association, and who have made significant contributions to their practice settings and the profession of pharmacy. These members have displayed the attitudes and attributes that make pharmacy the nation's most trusted profession. Please take a moment to nominate candidates for the awards described below.  NOMINATIONS DEADLINE: MARCH 1, 2024. 

 

ALL NOMINEES MUST BE A MEMBER OF LPA BY FEBRUARY 1, 2024 AND MUST BE LICENSED OR CERTIFIED BY THE LOUISIANA BOARD OF PHARMACY.

Please complete the following information so we may contact you for any further information about your nominations.
Please complete your nominations below.
* Must show outstanding representation of pharmacy by making significant contributions through LPA, civic and national activities (in that order of importance).

* Not Eligible: Incumbent President; Recipients of this award in the previous three years.
* Must have compiled an outstanding record of community service, which apart from his/her specific identification a a pharmacist, reflects well on the profession.

* Not Eligible: Previous recipients; Current Officers; Officers serving in the past two years.
* Must demonstrate excellence in Pharmacy communications, consultation and direct involvement with all members of the health-care team and patient.

* Must have made an outstanding contribution to the field of clinical pharmacy.
* Must have demonstrated innovative pharmacy practice resulting in improved patient care.
* Must be licensed to practice for nine (9) years or less.

* Must practice in retail, institutional, or consulting pharmacy.

* Must participate in national/state association activities and/or community service.
* Must have displayed creative pharmacy management (i.e. new ideas, expanded patient offering)

* Must have made technological advancements within the pharmacy.

* Must actively participate in new patient intervention programs
* Must be practicing in a chain store setting.

*Must have compiled an outstanding record that reflects well on pharmacy.
* Must be practicing as a consultant pharmacist.

* Must have made contributions to improving patient care and safety as a consultant pharmacist.

* Must have compiled an outstanding record of service that reflects well on the practice of pharmacy.
* Must have contributed to improving the image of pharmacy through compounding.

* Instrumental in education of both public and medical professionals about compounding and its value in patient care.

* Demonstrated skill in solving patient's medical needs through compounding innovation.
* Not Eligible: Officers serving in the past two years; Recipients of this award in the previous five years.
* Must be practicing in a hospital setting.

* Must have compiled an outstanding record that reflects well on the practice of pharmacy including successful accreditation by government regulatory agencies, involvement in quality improvement initiatives, and promotion of pharmacist-driven clinical services.
* Must have provided significant contributions together with pharmacists in the aid of pharmaceutical care.
* Awarded to one Graduating Senior from each College of Pharmacy.

*Must have attended at least one LPA Convention.

* Must have been exemplary student leader within LPA, through the Board of Directors and/or committee structure.
